Appointments
Appointment requests on this site — including requests sent through Modento, the online booking tool this office uses — are requests. They are not confirmed until our office confirms them.
If you have a true emergency, call 911 or go to the nearest emergency room. Our office phone is not an emergency line. For urgent dental questions during office hours, call (614) 891-6767.
Fees, cancellation, and no-shows
Payment and appointment fees at Sunbury Plaza Dental follow the office financial policy. This page is a summary. It does not replace the financial policy you sign at the office. If the signed policy and this page differ, the signed policy controls.
Balances are due on the date of service. If treatment is scheduled ahead of time, we ask for 50% of the estimated balance to reserve the appointment. The remaining balance is due before treatment.
We accept cash, checks, cards, HSA or debit cards, and third-party financing when it is offered. A 3% credit-card transaction fee applies to payments made by credit card.
Estimates are valid for 30 days. An estimate is not a guarantee that insurance will pay. You are responsible for the patient portion.
To avoid a cancellation fee, cancel at least one business day before your appointment, and do it while the office is open. If you cancel with less notice, we may charge $50 per hygiene visit and/or 20% of the scheduled restorative treatment.
If you miss an appointment (a no-show), we may charge $75 per hygiene visit and/or 30% of the scheduled restorative treatment.
Balances unpaid more than 30 days may accrue a finance charge of 1.5% per month. A returned-check fee may apply.
If you need a hardship arrangement, talk with the office before treatment. We review those requests case by case.
